- OT Assessment Planner: your name, district or organization, role, email address and phone number, together with the facility counts you entered.
- Bulletin requests and subscriptions: your work email address.

We use this information to respond to your inquiry, prepare a requested assessment scope, send the materials or bulletins you asked for, and follow up about our services. We do not sell or rent your information. We share it only with service providers that host this website and deliver form submissions to us, and only for that purpose, or when required by law.

This website does not use advertising or tracking cookies. If you switch between the light and dark display, that preference is stored in your own browser so the site remembers it.
